I am not sure about the number on the medical form but I got that fax number from a posting somewhere on this forum. You can also do a google search for fax number cic ottawa medicals.

I got my FBI report on the third try....what a relief!

1st FBI submission - 15th jan, paid 18 dollars thru credit card, got ink finger printing at local police station
2nd FBI submission - 13th march, did not pay anything, got ink finger printing at local police station again
3rd FBI submission - 16th may, paid 18 dollars thru MO, got ink finger printing at local police station (same police officer all three times, this time he gave a letter of support to say he followed all good practises as specified by the FBI website, and that they should contact the police station for any further queries)...

I submitted one fingerprint card every time, and received a FBI clear on aug 18th

waiting for medical request now so I can submit it....

No Stickers collected.......But the file number was there on the form and also Printed on the X-Rays

Thanks for the very useful info. So, if we get report from gyno then they don't have any objection ? My friend told me Ronald young was arrested on female complain. But she was not sure that guy was same.

Try Dr Wong in Bowery St. NY. The procedure was very fast and everything went smooth. The only drawback is you dont get a free parkin spot in NY :(. Also the fees was less.

Only minus point is we need to pay fee at each counter separately in cash.

Blood & Urine fee $30
Doctors fee $165
X-Ray fee $60

And for X-Ray, lab is about 5-10mins drive from this location.

Just wondering, for the guys that have already done so, what did the medical exam consist of? Just your average physical + xray? did you have to do blood test or urine test as well?
