The Consulate General of India-Toronto
Or
Consulate General of India -Toronto
Consulate General of India Toronto
Which one is right .Thanks

Below is the Mandatory declaration (Toronto Jurisdiction) for Minor OCI Applicant as well, same as Adult

Declaration by the OCI Applicant
With reference to my OCI application file reference number
…………………………, I hereby declare that I have surrendered my Voter ID
and Ration Card to the concerned Indian authorities in India after acquiring
Canadian citizenship or other foreign nationality.
If later on, I am found to have the possession of either of these
documents, i.e. Indian Voter ID card and Ration Card, I will alone be held
responsible for the repercussions of my actions of non-compliance.
(Signature of the OCI applicant)
Name:
Date:

Questions:
Document Upload: Does the signed mandatory declaration need to be scanned and uploaded to the OCI portal?
Name on Declaration: Whose name should be written in the declaration at the bottom - the minor applicant's name or the parent's name?
Minor's Signature: Is the minor applicant's signature required on the declaration if they are capable of signing?
Parents' Names: If the parent's name is required, are both parents' names required to be included in the declaration?
Parent Signatures: Do both parents need to sign the mandatory declaration for a minor's OCI application? Should there be separate copies for each parent, or can both signatures be on the same form, or is one signature from one parent sufficient?

For my minor child (born in Canada), this declaration was not needed as per Vancouver jurisdiction. Please confirm the same with Toronto jurisdiction

I have submitted my kid OCI application on 29th May at BLS ottawa.
Yes, the declaration has to be uploaded on the portal.
Both parents has to put their name and signature on a single/same form. The applicant name should be your kid name and his/her application number.
Do not forget to take a printout of Minor OCI consent form which again has to be signed by both the parents. Both parents photograph is required on this form and a cross signature on photo as well. This NOC form has to be uploaded as well on the portal.
Minor doesn't has to sign the declaration form.
If you are submitting in person, make sure both parents must be present at the time of submission. Minor doesn't need to go for application submission.
Both parents has to attest all the documents.
