June 2013 applications

Actually the test for my friends was end of November and their oath was December 6th.

I have applied for CBP report in online ( FOIA request ) few days ago as you suggested.

While applying i didn't upload any attachments like my ID/ any forms /written letter etc

I just wrote under Description by saying, I Need it for Citizenship and my date of birth & Passport number.

Do you think i should submit form with my signature and get it notarized before submitting ?

I think that should be enough. As long as they have your name, DOB, passport number and country of citizenship.
That is what I submitted online. I am still waiting for the report...

No I didn't

I have all my CBP stamps with the exception of couple however, I have my boarding passes as evidence of my travel and stamps back into Canada. It would be good to have CBP report if not I am covered.
