I just printed out the form and wrote there I needed it for citizenship application. I didn't include any fee.
They sent me the report after six weeks. I did that in August 2013.

Also, which option should I select under Method of access preferred: Receive copies of originals OR Examine originals in government offices

So just based on name and address they are able to provide the information?

To get your documents more quickly, write a letter, sign it, scan it, and send it along with a copy of a governmental ID to the following address : atip-aiprp@cbsa-asfc.gc.ca

It will take around 4 weeks to get your report!
 
I have applied for CBSA report in October and included a cheque for 5 Dollars. the cheque wasn't deposited and the report was sent to me on Friday. It took about 40 days.

2) Which option should I select under Method of access preferred: Receive copies of originals OR Examine originals in government offices

Just want to be sure with the verbage please.

Did the test today. It was really easy. Everybody who passed got a notice to appear the first week of January for Oath Ceremony. It's happening really fast! Good luck to everyone waiting.

sorry I just saw your message today. under section: you need to say" i would like to receive the history of my entries into Canada up until XXXX 2013.

you need to select receive copies of originals.

Thanks vias: I don't think all of the Ontario applicants are lucky at all. Mississauga seems to be a fast office. Some persons who did the test and were told to come back 1st week of Jan for the oath wanted their files transferred to Scarborough. The officers told them "sure - but it will probably taker another 6-12 mths for them to get to your file."

I'm not complaining at all. The Glen Erin office is very far but worth it. There were people there from Peterborough!

My friends who live in the same area as me in Toronto west applied April 2013 and they took their test in early December in Mississauga and a week later they went to their oath ceremony. That is quite fast for Toronto. It was only 8 months. They are both US citizens and they applied as a family. Their kids were born in Canada. They went to US very often almost once a month.
