IMM 5669 Schedule A - CIC asking for the form again, please Help.

Hi Guys,

I also got a Schedule A form to fill after the medicals passed. So, i am also not sure if i have missed anything, but i will be extra carefull in filling this up now and will try to be more precise.
I think you can add and modify info in this one as this is the reason they have sent it to us.Be very accurate in the entire information.

Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature covering the period of 2007/08 to 2009/07

Do I need to provide only for period 2007/08 to 2009/07?

What does that mean?

Yes, just provide for the period they have asked for and they want to know

Thanks Sir. one more thing, I have only mentioned the permanent addresses in the address history. is that fine?

Yes, that's fine. If you have stayed in one address for more than 6 months then you need to give Police Clearance for that place as well. So take care of that.

My college was in chandigarh and employment in Delhi. But i only mentioned permanent addressed there. My PCC is from PSK and it covers whole India.

Thats great. There seems no issue at all. Just be relaxed and send them the details as soon as possible. Your application will be processed really fast after Schedule A. All the very best..

I was officially employed by Accenture Philippines between January 2001 to April 2008, and I have a "Certificate of Employment" from Accenture Philippines supporting these dates which I can provide as a supplementary document, hence I believe it is simplest for me to place this particular employment activity as 1 row/entry in my form IMM 5669 Personal History section.

However, between the years 2005 and 2006, I have been assigned, as part of my employment in the above period under Accenture Philippines, to work for a client based in Peoria, Illinois (USA) for a short-term assignment twice - the first assignment was total of 6 months (around June 2005 to November 2005) and the second assignment assignment was total of 3 months (June 2006 to August 2006). Do I still need to bother declaring these 2 short-term assignments as additional rows/entries in my IMM 5659 Personal History section - if yes, then I would be required to provide FBI Identity History Summary (for the total of 6+3 months short term US assignments) apart from just NBI (police clearance from Philippines).

I got a email today asking for an UPDATED SCHEDULE A (IMM5669) YOU MUST DECLARE YOUR PERSONAL HISTORY AND ADDRESSES FROM THE AGE OF 18 (NOT ONLY THE PAST 10YEARS)
they should of explicitly asked for that, not like the pass 10years ot age 18 whichever is most recent.

Is there someone here who get a request to send a Schedule A but there’s no request for police check yet. I received an email asking me to send a schedule A only. Normally i can see people says they had a request on both at the same time. Is there anyone on the same boat?

I received PPR but i and my spouse have also been asked to complete an updated IMM 5669. The letter says that I must ensure that i answer question 8 and 12 (updated form has no question 12!) and must include all activities from 18yrs of age to 2017 without leaving any gaps. I am a little confused and wondering how is it that I got PPR yet they are asking me to submit this form.

I am also wondering what i did wrong the first time to get this request.... anyone else with this experience ?
