This thread has been very helpful not going to lie i was very anxious about how i was going to put everything together until i came across this.

A couple things iam still unsure of though, would it be a good idea for me to attach a cover letter in my application, i read that someone said that it is usally only used if their is missing informaton. I (Sponsor)as well as my husband both got our phones stolen last summer so all or main chat logs from may 2013- aug 2015 are missing, should i simply explain this in my cover letter? and provide proof ( eg. police reports of stolen phones)?

also my husband sent me photocopies of some of his documents (eg passport / school certificates) do these also need to be notarized?

I hope these arent silly questions..

thanks in advance for your help.

Photo copies of both. The only originals you need to send are police certificates. You will not receive anything you send, back. Do not send originals of the marriage certificate or birth certificates.

Hi there should photocopies of marriage certificates, birth certficates , school certficates and passports be notarized?

You only get a client ID if you had apply for a visa before, like a visitor visa, student etc.
If you don't have one right one, you will get one when you will submit your application.

My husband and i precious applied for a vistiors visa before we werre married but didnt get through, so do we need to include a client id and if so How do you find this client ID?

hello.

can I use my Maiden name in my forms since my passport still in maiden name.
what about status? it should be MARRIED even you use maiden name instead of your husband's name?
what about supporting documents? police clearance, NBI and medical? would it be okay to use maiden name too but MARRIED status?

your answer is well appreciated. thank you!

Hey Pelipeli

Thanks for putting this together! This will really help future applicants like myself.

Thank you very much, this has helped me a lot. Where did you put the receipt of payment ?

In a way, I wish I found this thread before I sent my application lol.

Now I'm kinda worried that we didn't provide much to prove the relationship. I'm sponsoring my husband who is a Japanese citizen, who was living in the US. We dated for 2.5 years and got married in August 2016. We answered all the questions and only provided a 1 page written explanation of our relationship (included timelines) and 10 pages worth of photos together (some with dates on it) of our ceremony/wedding, photos of dates... (2 pics each, pics of kisses/hugs, eating and an "Anniversary photo" and birthday photos). I also provided a copy of my insurance that he is one of my beneficiaries.

I tried not to provide TOO much 'cause I feel like I don't want to annoy the heck out of the clerk either...

I stapled things in an organized manner depending on what the forms and what is required to be provided. And I put it all the sponsor forms, sponsored forms together etc (not clipped together but in order) into 1 envelope. And for supporting documents, I provided a cover letter.

I'm nervous now haha

Hello everyone,

First off, I would like to thank Pelipeli and others for sharing their experiences here -

with regards to putting names on the form, I noticed that there is no option for putting in middle name

my full name for example is Betty Salazar Cortez

First Name (given name): Betty
Middle name (maiden): Salazar
Husbands Surname (Family name): Cortez

Should I put it as Family name: Cortez, Salazar Given name: Betty

passport shows the following

Surname: Cortez

Given Name: Betty

Middle Name: Salazar

Thanks in advance for your answers!
:)

It would be helpful if you gave the form number and the question number to be sure, but if they are not asking for a middle name, then do not provide it. Adding a name to your family name will just confuse them and the system. If I've ever had to give my full name when there are only two field, I combine my middle name with my given name - never my family name. i.e. Given name(s) - Betty Salazar Family Name - Cortez

Thanks for your response chilkootcee, as for the forms they are IMM 0008, IMM 5669, IMM 5540, IMM 1334 & IMM 5481
I corrected the forms just as you said but did not include the Middle Name - It now shows Family Name - Cortez & Given name(s) Betty

I really appreciate your help chilkootcee, thanks again! ~

Also, for everyone who will be sending their application to their sponsors,
please do not forget to include the last page of IMM 1344 with signature of the principal applicant.

Hi there, I would suggest you should put your complete name.
I did the same as chikootcee mention above . Combine your given name and middle name.

Cheers!

Thanks crazy D, I did as you and chikootcee have said, phew! I'm already having difficulty in the first line :D
I was actually planning to send our application tomorrow, went on sick leave so I can double check our documents,
I've seen so many errors plus I did not realize that I need to include my sponsored person signature on IMM 1344.
