as what leigh_anne, said, she also left the 2nd box blank, she's waiting for passport request, if im not mistaken, so tapos na ng VO review ng dox nya. i think di sya nagkaproblema don, so it might also work with our case.

dun sa timeline mo, yung in process mo yun ba yung date na nagchange yung status mo o yun yung nakalagay kung kelan nila start pinrocess papers mo?

in my case, my ecas changed to in status on july 20, 2010 pm (coz when i checked in the am, received by vo pa), and it stated there that they started processing july 13th. on july 22nd, i received my med request which was dated july 14th. Pa follow up mo sa lawyer mo dun sa post office.:-) but for sure, you'll receive your MR soon.

Parang ganito, kunyari:
De La Cruz
Maria Juana
Rosales (middle name)

... as they appear daw on the passport ayon sa instructions and part of our name in the passport ay ang middle name ..

wala naman naging issue ang CIO so wala din naman siguro magiging issue ang VO.

If wala middle name sa mga forms, dapat, sa lahat, wala na din middle names? like sa additional information na form. kasi dun, nakalagay lang, "name". at tsaka sa picture, diba susulat name sa likod, wala na din middle name dapat ilagay dun?

i read your case.... yung work experience mo ba nasa NOC for FSW pa din until now? if yes, if i were you, this is not an advice, opinion ko lang, i would withdraw my applications (FSW 2008 & quebec)... then file another application under the new rules etc... baka abutin ka ng apat hanggang anim na taon.... wag ka na rin kumuha ng agency, dito ka na lang sa forum... ako hindi ako nag agency, yung nagyaya sakin na mag apply anjan pa hanggang ngayun.... anyway, opinion lang you can ask nars... he's an expert...

Btw, you'll also be needing the photocopy of Permanent Residence Card when you pass the full docs

also, dun sa work experience requirement, nakalagay dun na include a business card of the signatory. what if kung walang business card yung pumirma o kaya hindi naman updated yung business card nya, ok lang kaya na di na ako magsubmit nun?

kung close mo iyong tao na nasa business card, i would recommend na magpaalam ka sa kanya at magpagawa ka na lang galing sa sarili mong pitaka ng updated business card niya. isa kasi ito sa mga requirement ng Canadian immigration e. kapag kumpleto lahat ng paper works mo, mas malamang na bumilis ang papeles mo.

in my case, i included my middle name because it was specifically stated "as shown in your passport or travel document". In item #1 (page 1) of the Application for PR form, I wrote my first names and middle name side by side on the space for "Given name(s)". In page 2, "details of family members", I also did the same for my husband and son.

In Schedule 1, item#1, I placed my middle name on the "blank" field below the "Given name(s)". In items #7 and #8, I also placed the middle names of my parents on the blank fields below the "Given name(s)".

I also did the same on Schedule #3.

BUT on the Additional Family Information forms, I wrote the names (mine, spouse, dependent child, parents, and siblings) with the MIDDLE INITIAL ONLY, starting with the first names and ending on the last names.

i included clear colored photocopies of my passport, as well as those of my family members. I had it notarized, though. As for the employment contracts, i only submitted a notarized explanation letter with supporting documents because we don't have contracts in my previous employers.

With regard to the business card, since my current employment is a family-owned corporation, the one who signed my reference letter is my sister who actually works as a clerk in our business. I just printed a business card for her with her contact details. I guess, the VO requires this in case they want to verify from your employers, so they have it handy and ready when they decide to call. So i think it's really a must that you include the business card.
