Yung sa 1, sabi mo keyed in date mo when you submitted the document to CIO. ano date linagay mo sa "To"? the date nung nag fill our ka ng application? kasi diba, usually, "present" ang ilalagay.

In regards sa addresses, namention mo kasi kung sa canada. Pumunta kasi ako canada 1 month, pero hindi vacation, pinadala ako ng company ko. ang visa na binigay was "WX-1 WORKER". And, nagstay ako sa hotel lang for 1 month. do i need to declare this sa "Personal History" and sa "Addresses"?

Please advice...

By the way, quick question: For Certified True Copies, specially Transcripts and Diploma, pwede ba hindi sa Notary? Sa school mismo kasi ako nagpaCertify True Copies nung TOR and Diploma ko.

yes if it is certified true copy no need na to notarize it

aliyah, target mo dec? sana magsabay tau sa PDOS! :) sana makuha mo na tlga yang medical para halos sabay sabay tau and hopefully may makasabay ako wait for visa and kasabaty sa mga seminars :)

puwede rin mag-partime ka. hehe... it took me 20 days to get my medical (june 2 ung letter, june 22 ko nakuha) kaya sipagan ang pangungulit ke postman :D

my first post here. FSW cat 1 din kmi ng family ko. si wifey ang principal applicant. 2005 pa initial submission nmin.
ang tagal na pala nitong forum na 'to and i just found it out last week. ang hirap magback track kasi 300+ pages na, grabe. anyway, itanong ko lang, kasi we also have our medical & rprf payment request na, plus some other docs pa silang hinihingi. as for the medical, nka-sked kami punta sa nationwide this friday. now, for the rprf, kuha lang ba kami ng chq/bank draft s bangko then isama sa submission nmin with the other docs na hinihingi ng embassy or merong "payment form" na dapat ding fil-apan at i-attach sa ibang docs nmin? sa general guide kasi may "fee payment form" na binabanggit kaso application fee lang nman ang nakalagay dun at walang rprf so we're a little confused.

for certified true copies, pls note on the following excerpt from form EG7 page 21

To have a photocopy of a document certified, an authorized person must compare the original document to the photocopy and must print the following on the photocopy:
 „„I certify that this is a true copy of the original document‟‟
 the name of the original document
 the date of the certification
 his or her name
 his or her official position or title
 his or her signature

Who can certify copies? People authorized to certify copies include the following:
In Canada:
 a commissioner of oaths
 a notary public
 a justice of the peace

Outside Canada:

 a judge
 a magistrate
 a notary public
 an officer of a court of justice
 a commissioner authorized to administer oaths in the country in which the person is living
