Urgent question lang po sa mga nakapagpadala ng passport sa CEM, dun sa appendix A kasi nandun yung application number box sa may FOR OFFICIAL USE ONLY, pero sabi naman dun sa instructions sa completed APPENDIX A please ensure that youhave clearly indicated your file number, your return address and all contact numbers for delivery purposes. Isusulat ko ba yung application number sa box?

Hi im not sure if understand your question correctly but my Appendix A form does not include "for official use only" statemet. What i did was I indicated the application number on the dotted lines as specified in the appendix A form located at the top left portion. see below for sample

I sent my passport to CEM on July 29 then received on 31. Where is your return address, manila or province? Tnx

sir did you write something inside the portion of application number (Appendix A), it says there for official use only, but on the other paper with intructions in filling up appendix A, it says there to ensure clearly indicated file number, your return address......
am i supposed to write my application number in this area?

Thanks but i think we dont have same appendix A. The one they send to me together with the email was a seperate sheet with a table with a title on top "Appendix A". And multiple rows and column for me to write the names and infos for accompannying members of my family. Im confused.

naku sir ragluf hindi po exactly ganyan ang receive ko, halos magkapareha naman, pero wala file number sa taas na nakalagay yun mga asked info yun pa din pero sa lower portion po nya may mga nakadugtong na na For official use only na may konting column din tapos sa mga column nakalagay: Remarks - Date received- No. Of passports IN - Date returned- No. Of passports OUT
Tapos sa upper right part po nyan tapat ng nakasulat na for official use only naka lagay yun space na Application number.

parang ganyan nga po, minus lang yung file number sa taas and yung portion na may signature ang applicant sa baba, tapos yun dalawang table na yan pinagkabit nila.

2. Saan ka pipirma? Me nakalagay ba saan ka pipirma?

- Dun sa box ilagay mo ang Application File Number mo. Yung For Official Use Only - relates to the whole form, hindi lang sa box. Walang nakalagay na "Do not write below this line, or do not write anything on this box" - so yung For Official Use Only is for the whole form.

- Siguraduhin mo me pipirmahan ka kasi kailangan signed and dated ang Appendix A.

Yun nga po sir, wala po portion kung san mag sign ang applicant, tinanggal ang portion na yun. At kaya alanganin ako sulatan yung application number. Kasama sya dun sa dinugtong na table kung saan nakatapat sya sa row na may nakalagay na For official use only.

Ok hinay-hinay lang. Best way forward as I see it, magtanong sa CEM - reply ka sa email ng PPR mo. Lagyan mo ang subject ng URGENT - INCONSISTENCIES IN APPENDIX A ATTACHMENT SENT IN PASSPORT REQUEST LETTER. Itanong mo ito:

1. The included instructions in your passport request letter dated <date> specifically mentioned to indicate the File/Application Number. There is no clear space/line in which to indicate/write the Application number and no instructions in the form as well as in the letter to where this should be affixed. Can you please clarify?

2. The included instructions in your passport request letter dated <date> specifically mentioned to sign and date the form as soon as it is completed. However, in the attached Appendix A form, there is no section, box or line indicated where the applicant can affix the signature and date the form. Can you please clarify where the applicant should sign and date the form?

3. Can you please verify if this is the correct form for Appendix A as attached in your message dated <date>? There are gross inconsistencies between the instructions and this form. Please see attached form included in your letter....(i-attach mo ang ipinadala sa iyong letter and attachment.)

Sasagot naman yan as soon as makita nila sila ang nagkamali at maaring magpadala ng copy ng ibang Appendix A na mas malinaw. At this point, it pays to be sure and steady - wag padalos dalos sa paghahangad na mas mapadali ang panahon. Malapit ka na sa finish line kaya sigurista na dapat....
