PCC requirements

I've received a Police Clearance Certificate from India. But it's dated weird Feb 2015 to Aug 2020 Saying that I live in India on those Dates. But I moved to Canada 2016. The picture on the certificate is very bad quality, can hardly tell it's a person. I don't really have time to get a new one but I'm scared to send this one in. I'm not sure what to do at all.

Is your passport issued in the year 2015?

My wife did her pcc recently and instead of giving the pcc certificate to her right away she was told that it would be send to her local police station. Though her first pcc done in February was given to her right away. Would anyone know what exactly might be reason behind it.
