When you go to the notary he will have a format. It's a common affidavit and they keep all formats with them.

Actually I went to the notary place today afternoon. But they don't have any templates.
I can't seem to find any template/sample online.

I have the same situation as Rohit but am currently in USA and want to make one and the same name affidavit.
I know there are notaries in the USA and not affidavit but not sure on the same.
Can someone please suggest:
1. What should I create: an affidavit or notary
2. Any online portals where the steps are for the same.

Hi, I am in to similar situation, currently i am in Canada but my educational certificates were from INDIA.
In my WES report they mentioned as below as my full surname is not there in educational credentials, only initial is there.

"The names on one or more of the academic credentials submitted for evaluation differ from the name indicated
above."

So, I have few questions,
1. like when do we need this affidavit to explain to CIC
2. Who should authorize this
3. Where should i get this done, I am in Canada currently, but my education certificates from INDIA

Please share your thoughts on the same, Thank you.

Hi, since you're in Canada, I'm not sure where and how to get it done. I can only guide you for India.

You have to mention as it's given in her passport.
 
Okks.. But in my pasport it is different thn her orginl name.. Dosnt it mtr??


Better to get it corrected. You have to put her correct name everywhere. You can try to explain it in an loe. But I don't know if that's acceptable or not.

Hi all,

Presently I'm in Canada and I've got an ITA today. In the permanent residency application (e-APR), I'm supposed to answer: "Has this person ever used any other name?"

The thing with my case is that my last name on the passport is Gadhavi and on my educational credentials, WES (in the section 'name on credential') and work experience letters it is Gadhvi.
In this case, considering the names only differ by an alphabet, Should I answer 'Yes' or 'No' to that question?
If I answer yes, what should I select for 'Type of name', birth or other?

Any suggestions would be much appreciated.
