Need some help in filling IMM5669 form

Hi,

I am passive user of this forum from some time and now I need your help and guidance. I am filling IMM5669 form and here is my questions.

Point 8: Personal history
It ask me about the all the places where I have worked. In this time period, I have assigned on some international assignments as well. During this period I used to go to different countries for roaming for 2/3 days. Do I need to mention this as well here? Also, the space provided in the application form does not enough to put entire information of past 10 years, it says "If you need more space, attach a separate sheet of paper". What dos this mean? Shall I attach one A4 sheet and provide all the information on that page? Please suggest.

Point 12: Address
My doubt is similar here as I asked in point 8.

Hi Tutu:
Regarding about space just attach a separate sheet of paper, the first row put your first and last name and your DOB:
Applicant name: XXXX Tutu DOB yyyy-mm-dd
second row, form and question you are giving answer:
IMMXXX "Question 8 work experience"
and add the info you need

Not sure about your intl assignments but if they were from the same company, just put them into that company lap of time

I think you have to mention all the places you have worked. Did you work for the same company but in different places, or for different companies?
I'm not sure about this one, maybe other users can chime in.

If you don't have enough space, write down the rest of your information (or I put all the information) in a separate sheet of paper. I think it could be written or typed. Just put your name and birthday on top of the sheet, write down the form number and the question number you are answering. In the form you can add a note saying that it's continued on the separate sheet attached and in this extra sheet you can put a note that it's continued from the form.

For addresses, I think it's for places you have lived for 6 months or longer. Double check that on top of the question.

Don't rely on my words, wait for others and see what they say.

I worked in same company but in different places. I went to Singapore on one assignment and then went to Malaysia as visitor for 2 day 2 times and once to Indonesia as visitor for a day. But confused on this whether I would need to mention these visits on my forms or not?

Thanks for explaining about the separate sheet. I would have to add separate paper for two or more item per form like in this case I would need to add extra item in case of question 8 (Personal history) and question 12 (Address). Can I put information related to both points in the same page or shall I put it in different pages?

Also as you mentioned "In the form you can add a note saying that it's continued on the separate sheet attached", do I need to write this sentence on main form itself or for this also I have attach one extra page.

Hi Tutu,
In IMM5669 form there is no option to enter day, only month & year mentioned. So i don't think you need to mention your 2-3 days visits. However you are going to mention these details in seperate form (IMM5562E). So you are cover here.
For extra sheet, this is what i did. I just copied that particular table in a word document & at the heading i put "Continuation of Section 8 (Personal History)" & then write down my details.

no need to mention like different thing, but you will mention it on your travel history like work travel

you can use the same sheet just put additional info for questions 8 and 12 and write your info for each with it own title

write "continued on the separate sheet attached" in the last row of questions 8 & 12

Hi Tutu,
Please read the form instruction carefully when filling any form. If you see instcuction here "List all trips you, and if applicable, your family members have taken outside your country of origin or of residence in the last ten years".
So you don't need to enter your trip to India or Canada travel.

No need to mention in question 8 or 12 but you must mention it on travels imm5562 and you explain there if were business or vacations

The instruction guide says ALL trips outside your country of origin or of residence, so yes if you travel twice to canada you have to put it twice doesn't matter if it is a 1 day trip, at the end of each paragraph remarks INCLUDE ALL TRIPS tourism, business, training etc...

no need to attest for example, copies of ref letter, pay stubs, wp and so
