no i submitted 2 forms . One signed by me & other signed by my wife but in her form she write details of her family but write my name as applicant not her name

I think this is a minor mistake, with CIC primarily to blame for not making the form clear enough. However, as long as you submitted two forms, two family details, and two signatures, I believe you're fine.

In form IMM5406-Additional family information form I couldn't get all birthdates of her brothers/sisters for 7 of them I wrote "unknown".

Will that be a reason for rejection?

I've sent my application package on 08.09.2014. Along with the package I had sent Notarized copy of Birth Certificate of my spouse (wife) with the English translation. However, I have now realized that the Birth Certificate was containing all the information like Date of Birth, Father's Name, Sex of the child born, Place of Birth etc. but the name of child i.e. my wife was not mentioned. Also, I have enclosed secondary school (10th grade) certificate of my wife elsewhere in the application package which clearly mentions the particulars like Name of my wife, Date of Birth and Father's name.

Should I send addnl documents?
