I got couple of questions:

1. My spouse is already Canadian Citizen, does she also need to do Medical as well? It's because in the website this is stated:
"You must have a medical exam before you can become a permanent resident. Your family members must also have one, even if they are not coming with you"

2. For landing, does she need to come with me?

No your spouse do not required to do medical, for every individual in application CIC sends a seprate form for medical request, in your case you should get one with your name only.
