Just got the "MEDICAL REPORT-Client BIODATA AND SUMMARY" (IMM 1017E).

should there be a separate BIODATA request?

the form you have is just medical request. biodata is a seperate form that comes with rprf (although latly it appears they are no longer doing this which makes sense it was a form that just ask your height eye and hair colour

I assumed that the biodata request was no longer necessary, because your photo should show your eye colour, and the medical report will attest to your height. That is, before August applicants reported having received biodata requests.

Just got MER forms for the entire family. Timeline is at the right. So happy now, next step forward!

The firm looks so simply than I expected, only 2 pages, is this right one? Also thought there is a barcode as others mentioned, but did not see it?

Congrats all who have received their MR today.. I just hope they have not made any goof ups in entering the correct email address, happened with cousin sent the email to incorrect email address (a typo)messed it up by 4 months

I have received MR around 2 weeks ago and RPRF received 3 days ago.

My questions are :

1) My spouse date of birth is wrong on IMM 1017 (E) Medicals document.
2) Looks like many of them received Biodata form along with RPRF request. But I did not receive any Biodata form.

Can someone please advise?

Thanks man, yeah, found the problem. Initially opened in IPad which showed the blank form only (probably fir security reason?), but seen the details with bar codes in my laptop. Thanks again.

Wrong DoB is certainly a problem. It must match the one in Passport. Please email CIC to get it corrected.
I didn't receive the Biodata form either. I think it should be OK. If CIC need this info, they will contact us.

Well, if you had received your AOR on the same email address, then it should not be a problem.

Lets hope for the best. We should get our MR's soon
