Question, I am the sponsor, my husband is the applicant. If I order for myself, you mean it will only show the notes for SA stage? So if I want to know about primary applicant (notes) I should order for my husband's name? Do I need a consent form for that?

If you select "Sponsorship" you will recieve the files on the sponsor only.

You want to select "Permanent Residence" to obtain the files for the PR app.

Yes, your husband must give you consent. The link for the form is in the post above.

Thanks ImABule! I'm assuming theres a place for me to upload the signed and scanned form then in the application?

I'll try and get on this later when I have a printer handy.

Section B and D remain blank.

Yes at the end before payment there is an upload option.

exactly.. i saw they requested welfare check only a few days after my AOR. the major delay is my stupid B.C social service.. cic had to asked the second request fax; before getting a negative result.
plus they have to check 3 members, my parents and myself; i guess that's why it took almost 90 days for SA for no reason. they could just tell me at the cic call centre or when i asked my MP. no, nobody told me this until i requested these notes.

Yeah gcms worth seeing the causes why our file got delayed
 
People who worries lot about a document error u made on your spousal sponsorship application should not worry too much as family sponsorship application is processed not like other skilled workers category and they will rectify minor mistakes
