I am new to this forum. I received a doc request from CPP-Ottawa asking for POF and my phD program information including these:
1. The field of study for your doctoral program,
2. The name and contact details of your doctoral advisor and/or academic supervisor,
3. A list of all publications that you have authored or have participated in.
4. A detailed CV
Do I need to get a letter from my department confirming 1,2,3 or I can write these information for them my self? Which email should I send these to?

For items 1 and 2, you could request a letter either from your home department or from the school of graduate studies. For 3 , you could make a list of all your publications, presentations and have your supervisor sign on it (I do not think this is mandatory but just to be safe). You know exactly what to do for 4.

In fact you could combine 3 with 4. It is a normal practice to include a list of publications as a part of your CV. Since they would like to have a CV as detailed as possible, you could just as well proceed in this fashion.
 
For those who submitted Canadian POFs. Did you get anything official from the bank, or just printed from your on-line banking?

For the proof of funds part, I got an official letter from the bank detailing the balance in each accounts. I got them to print out statements for the past six months and made sure these were stamped prior to mailing them

I did a print-out. The result was a 37-page long .pdf file. No stamps, no nothing. But I did make sure that my name, email, or mailing address appeared on each page of the document.

I printed my bank statements from online banking and took them to the bank to be stamped on each page. I also procured a letter on the bank letterhead certifying balance in the savings account where my settlement funds were deposited. Additionally, I submitted incoming wire transfer advice that detailed where the funds came from and for what purpose, etc (but, that was just because of my particular circumstances - you most likely don't have to have this last document).

1. Bank statement (recent, last 30 days, 6 months???)
2. Job letter (is the photocopy of the letter from supervisor ok? or I need to get a job letter from department again?).
3. any thing else?

Nothing official from bank....just got the printed copy of each month transaction history but stampped each pages.
