My test result is also in a sealed envelope, but it is addressed to me, so I broke the envelope. I guess it doesn't matter.

If it is sealed, you shouldn't break it. I had my CELPIP results in a sealed envelope. I called them and was told not to open the seal.

2) for paying the application fee, my credit does not have 9 months validity from now, can i send them cheque?

The 300 sub-cap is only for the specific NOC. If you apply for the PhD Stream, you'll still have to select a NOC, but you'll have a cap of 1000.

Question regarding the medical exam:

I suppose that people bring their medical chart and everything?

What kind of things can be used to refuse immigration status based on medical exam (I wonder because I made one ER trip and had a bunch of follow up exams that led nowhere, can this be used to refuse an application?).
