hi i am sponsoring my spouse in costa rica, my question is in the appendix a document checklist http://www.cic.gc.ca/english/pdf/kits/guides/3908e.pdf for my wife's PR forms, it says she needs the IMM 5540 which is a document that I fill up in my sponosorship forms... does this mean i have to fill it up twice? or is it just refering to my sponsorship app? thanks,
