Hi guys, just wanted to provide an update. I had my Oath last Saturday (4th) and went to try my luck at Passport Canada today. Luckily they found me in the system and I will pick up my passport tomorrow. Thank you everyone, especially chrysss for your time and advice, much appreciated!

Congrats for your new identity. Can you share what exactly they want to submit? should the guarantor come with us to the passport office?

No. The guarantor just needs to complete question 2 on the form and certify on the back of one of your passport photos(that it is your true likeness).
As for what to submit, it's the application form(3 pages), identity documents(original or photocopies - if photocopies, then your guarantor has to sign and date them to confirm he/she saw the original), original citizenship certificate. All originals will be returned to you. Please reform to the application form itself for detailed information.
