I just ended up with IRCC, and they told me all the assessments have been passed, even the security, but the criminality is still in progress. This is so weird because I know that security is the last assessment to complete, usually.

Last time I called my criminality was completed but security was in progress. This is the first time I came across this but I believe you're so close they will deal with that soon. Expect the final decision soon. This is a very good news for all of us Jan applicants

It's longer than the straightforward cases, but it is still within the range where IRCC's published guidance doesn't indicate that something has gone wrong though I think it's reasonable to contact IRCC now rather than continuing to wait indefinitely.
For a Protected Person PR application, family members outside Canada are processed through the appropriate visa office. IRCC's current instructions specifically say that family members abroad who are included in the application are processed as part of the application. Don't compare your spouse's medical situation directly with yours. Your medical exemption in Canada is a separate issue from the overseas dependent's processing.
My practical estimate would be around 1–4 months for the next substantive request (overseas), The overseas dependent file continues separately and the your applicantion can receive PR before the dependent's processing is finished.
