    Usually, when you are past your time for exclusion - you should be fine. Talk to a lawyer (Canadian immigration lawyer that's not from the pind) and see what else you need here. You are very likely going to be good, but avoid fraud/misrep/or anything like that in general.

    PPR/RFV is manually triggered. Also, we speculate some of an "online queue" that exists through which most people are approved. So, someone not being able to do issue a PPR/RFV due to being busy OR your position in the speculative "online queue" can be a reason among many others.
