If they submitted their application in July 2023 it is too early to have received PR unless they were part of the pilot program.     Agree with @armoured. IRCC will likely actually dictate what happens because the outcome of the TRV application(s) will narrow down your possibilities. If your spouse applies for TRV now and gets denied you gave no other option but to return to Canada on your own and start the sponsorship...

    It is not about being safe or not. There is likely a higher probability to get approved if you apply after AOR but a TRV is never guaranteed. You can apply for an OWP while doing Outland sponsorship but the applicant must be in Canada. It seems confusing but you can apply for Outland sponsorship...

    Depending the applicant the medical may be requested before or after AIP. Most applications are not even opened for over 1.5 years there is no rush to ask for a medical to be done. Has nothing to do with nationality.
