One and the same name affidavit

Hi guys,

I want to submit sibling relation ship as my sister has PR, but both father and mother names in our passports have discrepancies. I reside in USA . Does one and the same person affidavit work ? From where should I get the notary done ? Is it from Indian consulate or any USA notary services ?

Planning to get this format of affidavit

AFFIDAVIT

ONE AND THE SAME PERSON

I, the undersigned, <Name>, holder of <Country> Passport Number XXXXXX do hereby solemnly affirm and declare on oath that:

1. I am the deponent herein and a citizen of <country>.

2. I was born on <bday> in <location>

3. My brother is <name>, a holder of Canadian Passport Number: XXXXXX, and Overseas Citizen of India Card Number: XXXXXX

4. As a proof of our sibling relationship, I am submitting my Passport and my Brother’s Overseas Citizen of India Card to apply for my Canadian Permanent Residence

5. In my Passport ‘Name of Father’ is mentioned as <Name> and, in my brother’s Overseas Citizen of India Card ‘Name of Father’ is mentioned as <Name>

6. In my Passport ‘Name of Mother is mentioned as <Name> and, in my brother’s Overseas Citizen of India Card ‘Name of Mother’ is mentioned as <Name>

7. Therefore, the names <father name1> and <father name2> refer to the one and the same person. And the names <mother name1> and <mother name2> refer to the one and the same person.

So, I do hereby solemnly affirm that foregoing statements are true and correct. Hence this affidavit.

I got mine done by one on concession street in Hamilton. I also know there's one in Burlington. I think the one on concession is called Porter law or something.

Could you please let me know if i need to produce original documents to get “one and same person affidavit”? If so, what are all the documents are mandatory?

My passport does shows my all name in given name, and surname is blank. Should I explain in LOE or to provide the Affidavit?

Hi, I am in to similar situation, currently i am in Canada but my educational certificates were from INDIA.
In my WES report they mentioned as below as my full surname is not there in educational credentials, only initial is there.

"The names on one or more of the academic credentials submitted for evaluation differ from the name indicated
above."

So, I have few questions,
1. like when do we need this affidavit to explain to CIC
2. Who should authorize this
3. Where should i get this done, I am in Canada currently, but my education certificates from INDIA

Was it sorted. My wife had different surname before marriage and her educational documents & WES report have the same name mentioned. But, her current passport and other documents have the last name after marriage.
What should I submit as "Legal Name Change doc".

Please suggest.

Can anyone understand my case please,
It goes like in all my certificates and passport my father name is mentioned as "Suresh" it is uniform in all my documents but my father uses "Suresh Kumar" in all his documents. So i need to show his ITR's and Income as i want to apply for study permit , so will this be an issue and suggest me something if anyone has been through the same, do i need to have an Affadavit??

You can get your affidavit notarized in Canada. I did mine the same way.

The stamp should probably be of whichever country you've made the affidavit from. So if you got the affidavit made from India, it should be stamped from India.

That depends on your current location.. if you are in India, get it notarized from India, if you are in Canada get it done from Canada

The stamp should probably be of whichever country you've made the affidavit from. So if you got the affidavit made from India, it should be stamped from India.

Affidavit is something you type. Get it printed out in a pain paper and get it notarized in wherever you are. It doesn’t matter which country the document belongs to.
