URGENT Help Needed.

I received the request letter that asks for the following docs and states:

- Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. Please provide your personal history and addresses from the age of 18 to the present without any gaps in time. Please include any past visa refusals in Section 4. This must be received at this office by: 2020/02/12
- Police Certificate: Please provide an original police clearance certificate for Pakistan. This must be received at this office by: 2020/02/12
- RCMP Criminal Record Check: Please provide a certified criminal record check from the RCMP. From our records, it seems you are currently residing in Canada. If this is the case, please apply for an RCMP clearance certificate. If not, please let our office know so that we can issue you instructions on how to do so from outside of Canada. This must be received at this office by: 2020/02/12
- IMM5562: Supplementary Information - Your Travels form. This must be received at this office by: 2020/02/12

for the first item, there are 2 different IMM5669 on the CIC website:
1. IMM5669 (June 2019) this form does not have the requested information provision in section 4 instead it is in section 6.
2. eIMM5669 (Nov 2018) This form does have the requested information provision in section 4, but it says only to be used with IMM5289 kit or IMM5772 kit

Please help me figure which form to use.

Secondly, my father has stayed in UK and Canada numerous times as a visitor for less than 6 months. should I include the addresses he has stayed for those periods as his adrress?

I also received the smiler request from IRCC London.

- Use latest version of the form. I had the same confusion but I used the latest forms and they were accepted. Version of the forms are mentioned “left side bottom of the form.

- regarding visit/ travel history in supplementary form, yes you need to mention about each trip with city and country name , reason for the visit and month with year. Even he travelled multiple times to the same city and country you have to declare the same.

- if you’re father stayed in UK and Canada respectively less then 6 months (collectively all visits) send an explanation with current country of residence PCC, they will accept it.
In my file one of the dependent is born in Dubai but holds Pakistani passport but never lived in Pakistan, yes he visited multiple times to Pakistan and Canada, we submitted only UAE PCC and it was accepted with an explanation letter.

Thanks. This is helpful oinformation but I had already submitted the older version form going by the "section 4" comment. for RCMP, I have submitted a letter from my father that he has not stayed in canada for more than a few months visits and that he is in Pakistan right now.

A late update about my father's medical. We had the medical assessment done on January 23rd.. i received an email confirmation a couple of days later and then I received an email from Dehli Immigration Office asking for further medicl examination. The letter does not specify what kind of further investigation. We haven't gone back to the medical office as yet. The appointment is in next couple of days.

Hello guys! My mom can't do her sputum culture test but she keep trying! She did that many times already. Our deadline of their medical is February 17 .. do i need to inform my Local Visa office about this or the clinic does that? Anybody had experienced like this before? Thank you ..

Do you mean she could not produce sputum? Or could not get an appointment for sputum test? If it is the former, panel physician will use a specialied device to extract sputum from her. Either way, panel physician will inform CIC and regional medical office on the status of extracting sputum and/or delay in appointment.

She's struggling to do the sputum test..
We asked them if there have any alternatives way to finish that test .. They already rescheduled her appointment next week .
