I'm an Indian citizen in Canada. I want to apply for a PCC from Canada. The application form for the PCC is asking me to put my Indian address, however my family recently moved to a different address (which I have never lived in). Also my passport has my old Indian address on it (where currently no one is staying).

In cases like this what address do I put on the PCC form?

I looked at the BLS International FAQ and it says that if there is a mismatch between the address on your passport and the address on the PCC form, you can just submit proof of the new address (like your family's ID documents) with the PCC. Is this the right way to do it?

Put the new address of your parents. When you go to BLS, take as many docs as you can (of your parents too) as they'll ask for it. Take a printed copy of your mom/dad's passport, aadhar card, etc.
