I'm an Indian citizen in Canada. I want to apply for a PCC from Canada. The application form for the PCC is asking me to put my Indian address, however my family recently moved to a different address (which I have never lived in). Also my passport has my old Indian address on it (where currently no one is staying).

In cases like this what address do I put on the PCC form?

I looked at the BLS International FAQ and it says that if there is a mismatch between the address on your passport and the address on the PCC form, you can just submit proof of the new address (like your family's ID documents) with the PCC. Is this the right way to do it?

Put the new address of your parents. When you go to BLS, take as many docs as you can (of your parents too) as they'll ask for it. Take a printed copy of your mom/dad's passport, aadhar card, etc.

Cool will do that! But what happens after this? Do they just randomly go to that address and verify my identity? Or do they set up the time beforehand? Because my family is not always home so I'm worried if the police might make a visit there and there's no one home.

The PCC will follow the usual process, sometimes they call, sometimes they visit. If they visit and no-one's home, they'll call your parents to the police station or come again.

I really wished they had an option for putting multiple addresses on the PCC form. Instead of just making us give 1 address.

Best if you get a premium lounge service of BLS. Even normal applicants are sent back, you've got complications - they'll make you come back or pay more.

So I went there, and ended up paying for the premium service because they told me they cannot put my current Canadian address (since I don't have any proof of it). And they ended up creating a new PCC application on the spot and put my previous Candian address as the address because that is what is on my work permit. Will this be a problem? Because I don't live in that address presently.........but its in the same city and its just a few blocks away from my current address lol.

Pretty much the template to push premium lounge if there's even a tiny deviation from the norm.

I don't know what "problem" you expect, but I doubt it. As long as you are going to physically collect your passport from BLS, there'll be no other issue.

On the IRCC website it says "It is a serious offence to put false information on any of your documents/forms", so I'm wondering if they'll make a deal out of it by saying "You listed your previous address on your PCC even though you do not live there".
