If they say that proper document is missing... they might be right.
When you fill up ur document u need to have a checklist with you.
And you can add :
- previous work permit
- i.d : driver licence or Medical card if applicable + the brown paper.
- proof of address (if you feel like)
- NOD if applicable
- the proper filled and signed document.
