What to do if my employer refused to issue VJO?

Please help me with the requirements of the VJO. Is it only one form (A-0700-EF) or there are some other forms and documentation.
Please consider my question, I really need your help.

You need A-0700-EF, and it invokes some other documents. Such as your CV, company registration, financial record etc. Read the form carefully and strictly follow the instructions.

I dont think you would be able to do it all by yourself. The employer has to file for this and you would need the HR dept. signature.
