Dear seniors
I received a mail from CHC London stating..... "On July 26, 2010, this office sent a reminder to your consultant regarding the Immigrant Summary Information forms that we requested. These forms must be received by August 2, 2010".

I was wondering if someone could explain me
What 'Immigrant Summary Information forms ' are?

what is thhe purpose of them?

Why are they usually asked for?

Q3..... Please list all the countries which you have visited or lived in over the past 10 years. Give dates,Place, reason for stay and Immigration status.
I visited France for a week in 2006, Do i need to mention that?


Q 6,7.....Did you serve in army etc....If your answer is NO, why did you not have to serve in any armed forces?
What Should I answer for .... why did you not have to serve in any armed forces?
 
Q3 .. Why the confusion.. you visited France for a week so you should say YES. and put down the reason for your visit .. travel, tourist, visiting family or frient etc.

Q 6.7 You either say YES (if you had).. and then state your rank, division etc. OR you say NO .. and state that it is not a mandatory requirement for your country. In some countries it is mandatory to serve .. draft etc. in others it is not as it is voluntary.

This form is just similar to the background decl form u filled where u listed all ur stays since attaining 18 yrs. Remember it should exactly be the same info u provided earlier. This form is asked in duplicate . Reason i predict is a thorough background check for security clearance. All London applicants are not asked for this form. its a random call . see that u fill the same details as below it is mentioned that it is the same as u swear in a court of Law. So be carefull in the details part. Just browse SHIBLEEBD thread on Medicals RECD where u can figure out how many were asked this info.

I have another question please
In the background and other forms, my consultant didn't mention my 4 days visit to France....now in this form should I mention it or I should fill it out exactly like that of th old form

I can only assume your "consultant" is not an Authorised Representative, a Canadian lawer or CSIC member. I don't know why anyone would use these illegal "consultants" or "agents" when it clearly states at CIC websites who can legally represent you.

The problem now is that you are responsible for the accuracy of all the forms that your "consultant" submitted. You cannot say he was the one that filled up the forms as you will get a 2 year ban if they find out you used an unauthorized consultant.

Now I think the best option is to state the truth and if they call you for an interview explain that you made a mistake in the original submission. Maybe someone else had similar problem and suggest alternatives.

They need details of only a place u have resided 6 months and above. But I am not sure whether if they check ur passport later and find those entry & exits from France it could put u in trouble. As u have hired a consultant check with him abt his view point on this matter.

Well he is an authorised consultant of course and CSIC member as well. I suppose I'm safe now, isn't it?

well, i have 2 passports (old+new), france visa was issued on old passport which is now expired.
my question is....Should I send my old passport too? In UK. I never sent old ones for new extensions and just new pp is sufficient

If you have done ur entire process with the new passport no need to send the old passport across.
But if u have started with the old passport and inbetween renewed and got the new one then u need to submit both.

The working pattern differs from each visa post. Some visa office might ask for all passports held by urself with the ppr.

what are the chances of contacting my employer at this stage [my med was done mid June 2010 and sent to chc exactly after 4 days]
