Visa Refusal - Raise CSE?

In the question for visa Refusal I selected no.
I was refused a US visa around 10 years ago, (I applied a year later and have had 2 since then, with extensive travel history).

Should I be concerned?

Yes, you should have been truthful. Rectify the mistake and mention the refusal. Having a refusal should have no bearing on your application, but concealing the refusal sooner or later will.
