I did upfront medicals for cec back In july 2014. Today I received MEDICAL REPORT CLIENT BIODATA AND SUMMARY(IMM1017) from CIC. What should I do ?

Send them UMI number details with CIC IMM1017 form.

thanks for reply.

The imm1017 form has some fields need to fill by the doctor. so we called the clinic.

The staff from doctor's office told us, since imm1017 form filled and send by med office for upfront medicals, there is no need. just send the emedicals information sheet.

we have following documents, please advice which one to send..

1. imm1017 form received from cic
2. emedicals information sheets(contain UMI #).
3. receipt from medical office(contain UMI #).

Guys...need a suggestion....I am in Canada and my family is in India...can we do Upfront medical separately....I know I will not have any trouble here in Canada but since they are dependents, will the doctor in India be able to generate upfront IME # and perform the upfront medicals?

This happened to me as well. They asked for my wife medicals which i submitted a year ago.
so I called them and they told me that i have to do the medical test again because it is expired now.
