Submit old transcripts at WES without academic form

Hi. I am planning to get my evaluation done from WES. I have a set of old transcripts (almost 8 years). Completely sealed. Now 2 questions:
1)Can i go ahead and send them with my degree certificate copy. Or do i need fresh transcripts.
2) And is the academic form mandatory?

Hi..i am also in the same situation. Mine is 5 years old. Also the university had mentioned the date( july 2013) in the transcript with the seal n signature. Is it enough to get it verified by the controller and placed in the sealed signed envelope and send to WES. The first transcript which i got in 2013 took 3 months. Pls reply..wes not responding.
Academic record form is not mandatory to keep along with the transcript. You just need the sealed transcript from the uni and a copy of ur degree certificate which u can even put in another envelope and send to wes.
