Sponsorship PGP application summiting

Hi,

I have prepared documents for PGP (Parents and Grandparents Program) application, and waiting for re-open on Jan 2017.

Have some questions:
1) Should I pay the application fee before Jan 3, 2017?

2) What documents should be included in when I summit "fee payment form" (IMM5770)?

3) Is it acceptable to summit application in person to CPC-M at Jan 3, 2017?

4) Should I summit all documents at Jan 3, 2017 to CPC-M, or summit fee payment form with application documents (IMM 1344 and 0008)first, then summit the rest documents after I received the receipt of payment?

1) Include the fee payment form with your application.

2) You must include all of these documents together in one application package: http://www.cic.gc.ca/english/information/applications/sponsor-parents.asp

3) No. You won't be allowed to enter the building. You must use a courier.

4) Everything is submitted together.

HELP Please!
I want to sponsor both my parents.
Can I do just one application for both?
Or I need to send two different applications?

One application with one parent as the primary applicant and the other parent as a dependent.

The application package not including form "SPONSORSHIP EVALUATION" (IMM 5481). I am wondering if this form is necessary?

The application packaging for "Sponsorship of Parents and Grandparents" including some application forms. I am wondering all necessary documents are including in there or still need some of others?

That form isn't part of the parent/grandparent package.
