what should I do write her in applicant and if not how they will differentiate between mine and my spouse form?

In one form you write your name as Applicant, then father, mother, sons and others family members are related with the applicant, (You)
In the another form you need to write the name of your spouse as the Applicant, then the father, mother,sons and other family members are related to the applicant (in this case your spouse)

Don't get confuse with the word applicant.

Dear Spicey, Tracy Davies signed my nomination letter. I heard these days both managers (David Li & Tracy Davies) and in a very rare conditions their boss, Christa Ross are signing nomination letters. For 2014 only David Li signed those type of letters but this year they changed everything.

(May be) They think that Moving to a different place/address/work on exact month beginning is not realistic (which is true) in every change of address.
They asked me to mention the address detail from 2005 (of course i should have given for 10 years). Since there was no space i did not provide and i did not have time to fill additional form. So let us not forget that too when we send to cic. We can use the additional same form just to fill addresses section only correct?
