I feel I may find some useful information from this thread. Also, since many users posted recently are having their application assessed currently @ CPC-O.
Hence, I may find Useful and uptodate information.
My Case:
Applied on - Nov 05, 2013
AOR received - Dec 12, 2013
Married on - Apr 18, 2014

All I wanted to know is that after notifying CIC about my marriage what documents and forms do I need to submit them in order to add my spouse to my application.

Seniors plz help

First of all, you need to get your marriage registered and provide marriage certificate along with that update your forms with your spouse details. Also pay her application fee and send it to CIC.

Thnx Gary for response. I already got my marriage registered. Currently waiting for passport for my spouse. So, which forms should I update could you plz let me know form# (IMMxxxx) plz.

Controlled documents required for continued processing:



· Updated IMM0008 Application for Permanent Residence in Canada to be completed by the Principal Applicant

· A completed and signed IMM5406 Additional Family Information by your spouse

· A completed and signed IMM5669 Schedule A by your spouse

· A copy of your spouse’s valid passport

· A copy of your spouse’s birth certificate

· An original police certificate for all countries your spouse has lived for more than 6 months since the age of 18 years; (for more information, consult the following link:

· A copy of your marriage certificate;

· *If applicable*- A divorce certificate for your spouse, if he/she was previously married

In order to add your spouse to your application for Permanent Residence, the following must be provided for this person (in addition to the control documents indicated above):

To save on time send these at once:

(B) If your spouse is not a citizen or permanent resident of Canada, he/she must submit the following documents whether he/she will accompany you to Canada or not:


· Copy of the bio-data pages of current valid passport (Must include passport number and validity date)

· Completed application forms (enclosed):

· IMM 0008 Generic – All pages must be completed and page 5 signed by the principal applicant; pages 3-5 must show details of the newly added spouse, common-law or conjugal partner and all dependent children of principal applicant or spouse/partner.

o IMM 5669 Schedule A - to be completed by spouse, common-law or conjugal partner and all dependant children over age 18.

· IMM 5406 - to be completed by spouse, common-law or conjugal partner

· IMM 5409 - Statutory Declaration of Common-law Union if applicable

· Four passport-sized photographs

· Copy of your marriage certificate if applicable

· Copy of birth certificate (legible)

· Police certificates from any country where he/she has resided for six months or more since age 18

· Copy of divorce certificates/from any previous marriage(s)

o (If minor children are involved, custody documents must be included)

- You (the principal applicant) must complete the attached “Principal Applicant’s Relationship Assessment Questionnaire (Marriage)”. This questionnaire must be sent back to Case Processing Pilot – Ottawa as a hard copy (not as an email attachment).

- Your spouse must complete the attached “Spouse’s Relationship Assessment Questionnaire (Marriage)”. This questionnaire must be sent back to Case Processing Pilot – Ottawa as a hard copy (not as an email attachment).

- You must send evidence of the relationship between you and your spouse, such as:

· Proof of your spouse’s visits such as airline ticket coupons, boarding passes, copies of pages of your spouse’s passport showing entry/exit stamps



· Wedding photographs depicting all the rituals of a traditional wedding,



· Wedding reception and post wedding photographs,



· Evidence of continued contact with spouse such as phone bills, letters and e-mails and



· Marriage certificate issued by the Registrar of Marriages.

Photos must be loose. Do not send them in binders, albums, frames or other such containers. Do not send video discs or video cassettes. Do not send musical greeting cards or other similar documents containing electronic or mechanical devices. Do not send calling cards.

i did send these all above docs when requested and i had got the medical request the very next day they received these marriage docs, i was really lucky ,,, good luck

Hello People

I am in a bit weird situation here. Would appreciate any help.

Applied :- 6th May 2013
AOR Received :- 24th June 2013
Medical Request and RPRF request :- 28th Jan 2014
Did my Medical :- 19 Feb 2014
Left for India for my marriage :- 23rd Feb 2014
Passport Request : 6th March (I was still in India at this date)
Informed CIC about my marriage :- 7th March 2014
Received email from CIC to provide control docs for my wife :- 21st March
Returned to Toronto : 24th March (I came alone, my wife is in India)
Sent all control docs to CIC :- March 28 , 2014
Received Marriage questionnaire email from CIC :- 2nd April 2014
CIC received all supporting docs and marriage questionnaire :- 10th April 2014

After this date no email from CIC. On 1st May 2014 at 11 PM I got a "PPR email" again from CIC.
Now the confusing part here is that my wife has not done her medical yet. How can we get PPR without complete medical for my wife ?

NOTE :- After our marriage I did apply for a OWP for my wife from India on 12th March 2014. My wife did do a medical for OWP . She got OWP approved on 14th April.
She is scheduled to arrive in Canada on 6th June on OWP visa.

Now the only thing I can think of is why we got PPR without medical is :

1. May be I got PPR email by mistake
2. They automatically considered the medical result of my wife from OWP file and hence they did not request a separate medical for PR ?

Anyone faced this situation before ?

Yew they have considered the medicals from owp. So all is good and congrats.

Ur words definitely gives me some relief ... But have u seen any other case similar to mine earlier ?
In the meantime I have replied to CIC asking whether the process to add my spouse is now complete and should I submit both the passport at this moment ?

Still awaiting CIC response.

Should you wish to have your photos returned to you, please follow this link and submit a prepaid Xpresspost shipping label. Once the label has been purchased, please attach the PDF document to an email and send it to my attention. Upon receipt of the label, your photos will be returned to you.
