Quick PPR Qs?

My friend got PPR and have 2 Qs:

(1) Does she has to write her name on both photos? Or is it just a name and address of the studio on one photo only?
(2) On the bio-data page, does she has to write her Canadian child information also?

Applicant/person name on the black with ink pen and file/Application No only
I think it is written in letter that you do not need to send Canadian passport as he/she will use it to travel - Not needed
