In case you are from Pakistan, you need to present birth certificates as well as ID (NIC)cards now called CNIC. The older B / Bay Form contained the details of one's kids along with an identification no / NIC no. previously. Now after the arrival of the NADRA and the new Computerized NIC, it is called CNIC. NADRA also sparately issues B / Bay Form which contains the CNIC no. of the newborn child since birth. so every child has his NIC / CNIC no. since birth which has been mentioned in the B / Bay Form in the old as well as the new system. I have proof of both for one of my children which was registered in the old system but was later re entered to the new system with reference of the old NIC no.

As far as birth cert is concerned, very detailed and exact details have already been given.

From NADRA you can have your CNIC and the B / Bay form in URDU Language or in ENGLISH Language as you need.

thanks very much all of u,for ur valuable n useful suggestions.It means I shd enter the ID card # of my kids ,mentioned in the BAY form,instead of writing N/A.

101%, that is Identity of your kid. By not writing it if they try to search your NIC from Nadra, and they find that you have kid already holding an ID, they might assume you are giving wrong informations. Hope you are clear, i had my Son's birth cert, but still i applied for his CNIC to fill that space of NIC#, now waiting for his Bay Form. I thought i should go for it because he is already 6 months old.

I am Single and applying on my own (no dependents). So do I just have to enclose a copy of my NADRA ID card when asked for birth certificate?

You just have to give your NIC# in form, the number which is written on your NADRA issued card, you have to attached clear photo copy of your passport not ID card, only number is enough, 1st and 2nd page.

Nothing else.

Thanks Jawad. That answers my question. Having been born in the 80s when the system was different, the thought of going to the hospital again in 2009 to get the paperwork and then asking Mr. Nazim to sign it scares me. I hope the front pages of my passport and ID card no. answer CIC's questions.

You need to submit the following documents in my opinion:
1. CNIC card by NADRA in English or in URDU with its Translation in English - verified by a commissionar on oaths (what I
remember you are a Pakistani resident in UK applying from CHC London so possibly you should have a CNIC by NADRA in
English)
2. Birth Certificate from pakistan which is now avilable in computer printing biligual ( URDU / ENGLISH ). Your old birth certificate would make it easy to get a new one since you got Serial no. or birth record no. on it. you just need to apply for a new / fresh copy of it.

I am sure that both of these documents need to be submitted and be separate.

BC Guy, Leon, PMM & other seniors are requested toverify it.

Hi All,

If you do not have birth certificate for kids , you can always use school leaving certificate in lieu of birth certificate and for yourself, you can get a letter from a notary public attested stating your birth date and issued in lieu of birth certificate.
There is nothing to panic about it, we do this for lots of clients who do not have birth certificates, and we haven't had any troubles so far.

Please clear what exactly you need to know. That Nazim thing is not that hard if you want to take Birth Cert for your KID, Even if your passport have NIC# on it you must have to let them know you are holding ID card as well. Please refine your requirement. Hopefully we will reach to right answer.
