Question about Medical Clearance

Hi

I called to speak to an approved place to do medical exams for me and my children. I wanted to get some general information such as cost and how invasive the exam was (My 15 year old daughter is very private and was worried it may entail something she is not comfortable with lol) . This place is also 4 hours away. The receptionist was great and explained everything to me but she told me the only forms they have are one's for people entering the US. She said I would need to bring my own form. I can't find any form online that I would need. Is this something i will get from ICC when they request my medicals?
