Post-ita... joint account to change to my name... gift deed still needed?

I have a joint acc I hold with my Father which I'm using for my POF. I have gotten my ITA and intend to change the joint account name to mine. Do I have to get an affidavit written to prove that the money belongs to me (originally as well now)? If the account name is changed to my name alone, do I still need a gift deed from Father?
