Those companies who order the notes for you generally charge you $40 or so. You can easily doing it yourself or with some help from your family/friend if you are not in Canada.

It costs nothing if the PR applicant requesting GCMS notes is present in Canada ("Privacy Act"), and C$5 if someone else in Canada is ordering it for the PR applicant ("Access to Information Act"). Tick the corresponding box in the following form:

www.cic.gc.ca/english/pdf/kits/forms/IMM5563E.pdf

Generally "Electronic Files" (printout from the computer system) is good enough to check what's going on with the file in process. Avoid "Physical File" because it means retrieving the content of the physical file at the visa office which will cause delay to your file's processing.

7-Mar-2011 AOR

4-Apr-2011 In Process

3-Apr-2012 AIP

3-Apr-2012 DM

18-May-2012 PR/Landing

My medical was done around the 12th december 2010, so dont worry too much.
I too was very worried about my medical expiring, the local CIC offices told me they tend to let the 1 year slip if there is no apparent problems with the 1st medical, so just wait for them to request another.
