I need help regarding Birth certificate Affidavit for my mom. I would like to know whether it will be provided by herself or by her relative who has witnessed her birth. And can someone please help me with the format of this affidavit.
2. Does she still need to provide a marriage certificate affidavit as well as my father passed away last year.

It varies from country to country. But the easiest way to get is a Notary Authorized birth certificate. The affidavit is usually prepared by lawyer and it is very standard format mentioning,

1. The name of the person
2. parents name.
3. Date, Time and Place of birth
4. Attested/Stamped by the lawyer/notary

Sorry to hear about your dad. Regarding the marriage certificate I am not sure, your best bet is to call CIC and ask that question. And if you can get a Marriage Affidavit as well it can't hurt if you provide it.

In this case, the marriage certificate is not necessary. However, death certificate is required. Who is the principle applicant? Is your mom or dad?

Yes. A marriage certificate (or an affidavit in its place) is required even if your father is deceased. In addition, you would also have to provide a copy of your father's death certificate. All the best on your application!

I have the same question too. Within the government, the first month is April until March of the next year. When CIC release this data to the public, is that from Jan - Dec?
