Yes - Medical expired after 12 Months - Hopefully IRCC will make a request for a fresh Medical Examination

IRCC tries their best to wait until within 1 year of approval so that parents don’t keep on having to repeat and pay for medical request especially when processing can be multiple years. Not having a new valid medical will not stop processing.
 
Same for me application was submitted on Friday Aug 2 2024,
Biometrics done was done 2025 but no update after that
We submitted Application Aug 1st. We received prearrival letter on June 24 , 2026. No medica so far.
What is pre arrival letter for? Showing any milestone in application?

Prearrival letter is purely procedural and can be received at any point after eligibility. The name is very confusing and the letter should be renamed because most people incorrectly assume that means that approval is coming soon.

Medicals are only valid for a year so IRCC does try to minimize how many times they request a medical. Fairly common to have to do medicals at least twice but one of the reasons that medicals are not ordered right away. It makes no sense to order them too early and have to pay to review them and applicants pay to keep repeating them. In a senior population health can change very quickly so you do want to order them when the application is actively being processed and not just sitting somewhere in a large backlog. Processing time is years for PGP and now that quotas have been reduced so processing times will go up (or reflect posted processing times) and we are unlikely to see many get processed in around a year which was happening fairly frequently for a few years. As long as your timeline is similar to others all you can do is wait.

Hopefully you will receive medical req in a week. We received the prearrival letter last week.

Prearrival means very little when it comes to processing time and it is purely procedural sometime after eligibility. There is a good chance parents may also need to complete a 2nd medical depending on processing time. PGP is a long process. There was brief period where processing became faster for some but in general it is a slow process and not IRCC’s priority because it rarely is a net benefit to Canada like economic immigration and spousal sponsorship/sponsorship of dependent children. Even if it takes multiple years to process people are still incredibly lucky if their parents eventually get PR. I would suggest monitoring your accounts but not becoming too obsessed with monitoring progress because there are very long periods of time where nothing happens.
