Ordering notes. How and worth it?

On the consent form in the first part is that ny name or my wife's name. And part 3 do we leave that blank? Has anyone filled this out before?

SECTION A

Name of the Person giving consent
needs to sign as well.

(If information is being requested for your wife - it will be name of your wife and she has to sign it)

SECTION C

Name of the person who is being authorized to make the request

(The person being authorized must be a Citizen or a PR or a person who is residing in Canada. A Citizen or a PR need not to be residing in Canada when making the request)
