open work permit application

hi there!just new here in the discussion forum and hoping to get some remedies and thoughts on how to apply for a permanent residency within canada...
i am married for a year now to my husband who is a permanent residence already here in canada under the PNP program.and i came here all the way from israel as a caregiver under the live-in caregiver program....so i have a valid work permit as caregiver until 2014, we planned to apply for a spousal application and hopefully it was all done next week cause i have to take my medical exam this monday and after that i may able to send my application in vegreville already, my question is...

1. i want to change the status of my close permit as a caregiver into open work permit?
2. and what will i'm gonna choose in the IMM 1249 on what i am applying for?
a. should i check the initial work permit under letter C OR
B.should i write beside the question what i am applying for? the OPEN WORK PERMIT?

hope you can give some ideas also about spousal sponsorship from within canada.
