Only initial as last name in Birth Certificate

Hi Everyone,

I am sponsoring my wife for the Spousal Sponsorship visa. Her full name mentioned in the passport is L****A V********N.

Unfortunately, all other documents which include her Birth Certificate and educational certificates only have the initial as a last name: L****A V.

Should we apply for a new birth certificate or get a notarized affidavit mentioning her full name and apply with the existing birth certificate?
