OHIP residency rule

I plan on landing as PR later this year. But I may need to be traveling back and forth between Canada and the US during my first year (because of family issues). I'm worried that I may be gone for more than 30 days during my first six months. As a new PR, if I apply for health card but don't use any health care services during my first year on OHIP, will they really check to see if I meet the 183 day residence rule? And if they find out, what's my penalty if there is nothing for me to pay back?

No penalty if you don't use services but you have to inform arrive Canada and start again.
