I currently reside in Canada, but am applying outland in response to the better processing time. For people in this situation, the applications are processed in Ottawa. (It would be processed there for me anyway as a US citizen, but I have "double" the reason to be processed there.) I'd be content applying inland, but 14 months for stage 1 approval (and resulting open work permit) is far too disruptive.
Inland applications aren't assigned to "visa offices", of which Ottawa is technically one.

I don't have any evidence that I was being given a run-around. The call centre operator can't make decisions, they essentially exist as an interface between CIC internal files and the applicant or representative. This is vital both for data-protection reasons (so as to keep anything that's supposed to stay confidential actually confidential) and for human-relations reasons (since the vast majority of applicants are not well-versed in the system).
The operator noted that the situation was unexpected, told me that my file appeared to be being processed as it should be, marked the irregularity for someone with an appropriate authority to deal with, and gave me an approximate timeline on when to follow up. That's precisely the best I could reasonably expect.
