Missing documents Help in family Sponsorship !!!!

Hi All

I wanna ask you for a favor i don't know if anyone can help me or not.

My wife is a Canadian and i have applied for a PR since she will sponsor me.

I have sent all the documents and forms requested. But yestarday i recived an email that some forms are missing.

What i have understand from the mail that i am missing only this form [IMM 5546].

I am asking if i can send to anyone experienced and just read it and confirm the same or something that i miss.

It will take 5 minutes but it will help me a lot.

I also can't send it by email or i have to send it through DHL ?

cutally, just googled it...it's military service..hmm...we didn't have that one to fill out. I guess it varies from country to country...
