Letter of Authentication for G driving licence

Can anybody suggest me a format of "Letter of authentication" from RTO in Bangalore, India for driving experience as I have more than 3 years of 4 wheeler driving experience. My e-mail id is ram.shubhu @ yahoo.com please send me a sample of "Letter of authentication".
